Voluntary Pag-ibig Membership -- Documents to Bring When Applying

On the Pag-ibig Fund website, one is instructed to bring only one valid ID for voluntary membership application. But when I went early this year to the Makati Pag-ibig branch along Gil Puyat (formerly Buendia), I was given a list of documents to submit, depending on my current employment or unemployment status.

Here are the documents required for applying for Individual-Payor or Voluntary Membership, depending on your current employment or unemployment status, according to a printed list given by Pag-ibig.

Unemployed who are former employees and former Pag-ibig members
- Affidavit of unemployment
- Certificate of remittance of contributions
- Two 1x1 ID pictures

OFWs who are former Pag-ibig members
- Latest contract of employment
- Certificate of departure
- Passport or valid ID
- Two 1x1 ID pictures

Self-Employed Professionals
- PRC/Bar license
- Latest ITR
- Certificate of remittance of contributions
- Two 1x1 ID pictures

Self-Paying Employee Who Is Waiving Membership Thru Company
- Notarized certificate of employment and compensation
- Latest payslip
- Company ID
- Certificate of remittance of contributions (for former members)
- Two 1x1 ID pictures

Self-Employed with Business
- Latest ITR with financial statement
- Certificate of remittance of contributions (for former members)
- Any one of these: business permit, mayor's permit, SEC registration, DTI registration 
- Two 1x1 ID pictures

Self-Employed as Transpo Operator 
- Franchise permit
- OR/Car registration
- Latest ITR
- Two 1x1 ID pictures

Self-Employed Earning Commission Income
- Latest ITR
- Notarized certificate of employment with average commission
- Company ID
- Certificate of remittance of contributions (for former members)
- Two 1x1 ID pictures

Unemployed Wives or Husbands of Pag-ibig Members
- Written consent from member-spouse 
- Certificate of employment and compensation of member-spouse
- Affidavit of unemployment
- Two 1x1 ID pictures

Note: You can get your certificate of remittance from your former employer or from the Pag-ibig branch where your former employers have submitted your contributions.

  2. Hi johndelfer, I think now they don't require this unemployment affidavit strictly. I think your IDs are enough.
