Voluntary Pag-ibig Membership -- Documents to Bring When Applying

On the Pag-ibig Fund website, one is instructed to bring only one valid ID for voluntary membership application. But when I went early this year to the Makati Pag-ibig branch along Gil Puyat (formerly Buendia), I was given a list of documents to submit, depending on my current employment or unemployment status.

Here are the documents required for applying for Individual-Payor or Voluntary Membership, depending on your current employment or unemployment status, according to a printed list given by Pag-ibig.

Unemployed who are former employees and former Pag-ibig members
- Affidavit of unemployment
- Certificate of remittance of contributions
- Two 1x1 ID pictures

OFWs who are former Pag-ibig members
- Latest contract of employment
- Certificate of departure
- Passport or valid ID
- Two 1x1 ID pictures

Self-Employed Professionals
- PRC/Bar license
- Latest ITR
- Certificate of remittance of contributions
- Two 1x1 ID pictures

Self-Paying Employee Who Is Waiving Membership Thru Company
- Notarized certificate of employment and compensation
- Latest payslip
- Company ID
- Certificate of remittance of contributions (for former members)
- Two 1x1 ID pictures

Self-Employed with Business
- Latest ITR with financial statement
- Certificate of remittance of contributions (for former members)
- Any one of these: business permit, mayor's permit, SEC registration, DTI registration 
- Two 1x1 ID pictures

Self-Employed as Transpo Operator 
- Franchise permit
- OR/Car registration
- Latest ITR
- Two 1x1 ID pictures

Self-Employed Earning Commission Income
- Latest ITR
- Notarized certificate of employment with average commission
- Company ID
- Certificate of remittance of contributions (for former members)
- Two 1x1 ID pictures

Unemployed Wives or Husbands of Pag-ibig Members
- Written consent from member-spouse 
- Certificate of employment and compensation of member-spouse
- Affidavit of unemployment
- Two 1x1 ID pictures

Note: You can get your certificate of remittance from your former employer or from the Pag-ibig branch where your former employers have submitted your contributions.

  5. When I stopped employment, I continued my contribution the following month as voluntary but did not submit any affidavit of unemployment. Is this requirement only for cases when you have been unemployed for a long time and for how many months/years?

    1. Voluntary Pag-ibig Membership: What you did was correct. You can continue paying as voluntary without any additional paperwork. They issued these requirements years ago, and they have not been implementing it. I will edit the above post later.
      Their instruction though is to make your first voluntary payment at your branch so that your next payments, whether you pay at Bayad Center or any other payment channel, will be posted to your branch. Anyway, just keep your receipts so you can countercheck your total contributions with Pag-ibig's total later on.

  6. This is my scenario. I just got accepted sa company, wala pa ako nag start work kasi nag cocomply palang ako ng requirements, isa sa requirements ay PAG IBIG, my question is wala po akong past work/business, wala po akong ITR or anything na nasa proof of income requirements, paano po ako mag eenrol?

    1. pag-ibig registration: Itong mga instructions dito ay noong very strict pa sila. Mag-enroll ka na lang online. Punta ka sa pagibigfund.gov.ph then click e-services, then member registration. Bibigyan ka ng registration tracking number. Temporary ito. Later on, bibigyan ka ng Pag-ibig MID. Kung wala kang text na matanggap, call 724 4244. Merong option na MID number. But if makadaan ka sa Pag-ibig nearest the company kung saan ka mag-work, punta ka na lang doon with your ID para makakuha ka na rin ng member's data form (MDF) para mai-submit mo sa employer mo. I-check mo na yong "Employed Private" kasi new employee ka na. Kung tanungin, sabihin mo na wala ka pang company ID kasi newly hired lang.

  7. hi... i just stop working and i have an existing mpl salary loan... just want to ask what to do and the requirements needed if i will apply for reloan on my existing mpl? i will be paying my contribution on march also as voluntary... can you help me what to bring at your office? thanks a lot...
